Case Management
The following services are offered through the Case Management department:
●Assistance with completing Advance Directives
●Crisis intervention
●Discharge planning
●Support groups
●Support services for patients and family members
The case manager assists patients and family members with admission, develops a care plan to address psycho-social needs, provides non-medical aspects of care, helps coordinate home care for patients following discharge, and acts as the "patient advocate" should a concern or problem arise.
